What is Lovenox (Enoxaparin)?
Lovenox, also known as enoxaparin, is a low molecular weight heparin (LMWH). It is an anticoagulant that helps prevent blood clots by slowing down clotting factors. Many patients receive Lovenox after surgery.
Nursing considerations:
- Monitor for excessive bleeding, including:
- Gums
- Nose
- Gastrointestinal tract
- Unusual bruising
Supplies You’ll Need
Before starting, gather all necessary supplies:
- Pre-filled Lovenox (enoxaparin) syringe
- Alcohol prep pad
- Clean gloves
- Sharps container
Medication checks:
- Confirm the medication and patient name
- Verify the correct dose
- Check the expiration date

Step 1: Prepare the Medication
- Perform hand hygiene and don clean gloves.
- Open the syringe packaging by peeling off the back label. Do not push the syringe through the packaging, as this could damage the needle.
- Remove the syringe by grasping the middle of the barrel, avoiding contact with the plunger.
Key Parts of the Syringe:
- Barrel: Displays drug name and dose. Double-check both.
- Air bubble: Keep the air bubble; it helps push all medication in and reduces bruising.
- Needle cap: Keep on until ready to inject.
- Plunger: Push to administer medication.
Store the prepared syringe safely until ready to inject.
Step 2: Select and Prepare the Injection Site
Location:
- Subcutaneous tissue on the abdomen, at least 2 inches away from the belly button.
- Tip: Use three fingers width from the belly button as a visual guide.
Clean the site:
- Use an alcohol prep pad.
- Clean in a circular motion from the center outward, covering about 2 inches.
- Allow the site to air dry completely. Do not wipe, blow, or wave dry.
Step 3: Pinch the Skin
- Use your non-dominant hand to grasp a fold of fatty tissue between thumb and fingers.
- Maintain this pinch throughout the injection to prevent injecting into muscle.
- Release the pinch only after withdrawing the needle.
Step 4: Inject the Medication
- Remove the needle cap.
- Hold the syringe like a dart with your dominant hand.
- Insert the needle at 90° for most patients.
- For very thin patients with little subcutaneous tissue, adjust to 45°.
- Use your thumb to depress the plunger, injecting both the medication and the air bubble.
- Withdraw the needle at the same angle used for insertion and then release the skin.
- Engage the needle safety.
- Do not rub or massage the injection site.
Step 5: Dispose and Document
- Place the syringe directly into a sharps container.
- Remove gloves and perform hand hygiene.
- Document the administration according to your facility’s policy.
