Scenario: |
A 73 year old female presents to the ER after her neighbor found her lying in her driveway. According to the patient’s neighbor she found the patient lying in her driveway and seemed to be incoherent. On assessment you note a 4×4 skin abrasion on the patient’s right knee from the fall but other than that the patient is okay physically. However, the patient can not recall what she was doing outside or how she fell. When getting a health history from the patient, the patient is not able to recall what medications she takes and rambles between topics. Ten minutes later the patient’s daughter arrives and informs you that her mother has been forgetting things lately and it seems to be getting worst. Last week she stated her mother called her and asked her to take her to the grocery store because she was out of milk. The daughter states she told her mother that they went to the grocery store yesterday and she bought milk. When the daughter arrived to mother’s house to show her she bought milk she found the milk underneath the sink from where her mother misplaced it. The daughter shares she does not want to put her mother in a nursing home but keep her at home for now. Pt VS: HR 80, BP 120/80, O2 Sat. 98% on RA, RR 16, and Temperature 98.6.

Nursing Diagnosis: |
Impaired memory related to central nervous changes secondary to degenerative brain disease as evidence by reported experiences of forgetting and inability to recall recent events.

Subjective Data: |

Nursing Outcomes: |
-Pt will identify 3 techniques to improve her memory.-The patient will be free from any injuries while in the hospital.
-The patient will be set up with an agency that provides a 24 hour sitter or caregiver so the patient can continue living at home by discharge. -Pt will be reoriented when confusion presents itself. -The patient will be encouraged to be as autonomous as she can be activities of daily living. |
Nursing Interventions: |
-The nurse will demonstrated 3 techniques to the patient and daughter on how to improve memory.-The nurse will educate the patient about using the call light for help and utilize the bed/chair alarm.
-The nurse will consult with discharge planning on setting up with an agency that provided 24 hour care givers at discharge. -The nurse will reorient the patient when the patient presents with confusion. -The nurse will encourage the patient to be independent in activities of daily living. |
