A patient is considered non-compliant when they desire to comply but cannot due to certain factors preventing them to do so. The preventing factors can be a number of things from situational barriers due to financial issues to impaired ability to complete a task due to a disability.
Nurses will find in almost everything setting a patient who is non-compliant. It is the nurses goal to help educate and provide the patient with resources so they will have the opportunity to comply.

A 72 year old female is admitted with Congestive Heart Failure (CHF) Exacerbation. According to the patient admission records the patient was just discharge for the hospital a week ago with the same diagnosis. The patient medications from the discharge medication list included: Lasix, Digoxin, Cardizem, and Lisinopril. You ask the patient about her medications and she presents to you the same list you retrieved from the electronic health record. The patient admits to you that she has not taken these medication since she was last in the hospital which was a week ago. The patient gives no explanation. However, when you ask her why she hasn’t taken the medication since then, she hesitates and states that she can not afford them. She states that she wanted to tell the nurse who discharged her last week but was too embarrassed. The patient states that if she could afford them she would take them as prescribed.

Noncompliance related to barriers to access secondary to financial issues as evidence by verbalizing unable to afford medication and progression of disease process (congestive heart failure exacerbation).

-The patient will form a verbal contract with the nurse that she will take her medications after receiving financial assistance with them. -The patient will verbalize 3 risks and 3 benefits of adhering to the medication regime. |
Nursing Interventions: |
-The nurse will notify the hospital social worker to help set the patient for financial medication assistance.-The nurse will educate the patient how and when to correctly take her medications.
-The nurse will assess the patient for any other problematic factors that the patient may face with medication adherence. -The nurse will form with the patient a verbal contract that the patient will take her medications after receiving financial assistance with medications. -The nurse will educate the patient on 3 risks and 3 benefits of adhering to the medication regime. |
